Health Checks has been developed specifically to comprehensively support PCT, General Practice & Community needs for the delivery and on-going improvement of a successful Health Checks programme. All current guidance is implemented and is customisable by the PCT, including extensive risk calculation support for algorithms such as Framingham, ASSIGN & QRISK2.
For GP Practices
Health Checks supports GP practices tasked with performing GP-led Health Checks. Because identifying patients eligible for Health Checks is onerous and time consuming for practice staff, Health Checks automates this process. Health Checks supports complete cohort identification, call/recall and Health Check consultations.
For PCTs & Commissioners
Health Checks supports PCT Health Checks management, consultations and reporting for GP or community led approaches. The software can be completely customised for local enhancements to national programme and allows PCT management via web-based dashboards. It provides organised recalls by date of birth or priority based on local requirements and supports structured data collection, patient education and recording of advice and referral given to patients in the clinical system. It also allows one-click e-referrals with custom templates to local services.
